Personal Care & Home Support / Daily Living

When it becomes clear that either your loved one or parents are struggling with the day-to-day tasks of maintaining their own personal hygiene, dressing, toileting, their own appearance, cleaning, gardening, shopping and attending personal appointments, it can be an unsettling time for families. Personal care can be difficult for family members or those close to you to carry out, particularly the more intimate tasks or those that require specialist clinical intervention, like changing a catheter or maintaining a stoma bag.

At Aura Nursing Agency our expertly trained carers can provide sensitive and respectful personal care focused on ensuring dignity and independence, in the familiar surroundings of your own home. Our highly personalised care plan will capture all your needs and particular preferences including how, when and where you would like the care and support you need, working together with your routines and lifestyle. There really is nowhere better to receive personal care – than in your own home.

Personal care at home is a very broad term used to describe a number of different areas of care & support that a carer provides, and it includes but not limited:

Meal preparation support service:

  • Planning Menus
  • Preparing Meals, cooking and serving
  • Monitoring and stocking pantries

Our support workers are available to visit your home in the mornings, afternoons or evenings to prepare healthy meals to suit your tastes using ingredients and recipes you desire. It's your decision how little or much support is enough. Our friendly and experienced support workers will accommodate your meal preferences with a tasty and reliable food service you can rely on.

High Intensity Nursing Care

High Intensity Care supports are where specialised, more skilled or more experienced support workers may be required to deliver supports to some participants who need it, Aura Nursing Agency provides,

  • Eteal (Naso-Gastric Tube - Jejunum or Duodenum) Feeding and Management
  • Tracheostomy Management
  • Urinary Catheter Management (in-dwelling Urinary Catheter, in-out Catheter, Suprapubic Catheter)

  • Subcutaneous Injections
  • Complex Wound Management

Community Care Service

Community care services are intended to help people who need care and support to live with dignity and independence in the community and to avoid social isolation. The services are aimed at the elderly and those who have mental illness, learning disability and physical disability. The main aim in providing community care services is to enable people to remain living in their own homes and to retain as much independence as possible, avoiding social isolation. Local authority social services provide community care services or arrange for them to be provided.

There is a wide range of services that may be available, including:

  • Home care services: help with personal tasks - eg, bathing, washing, getting up and going to bed, shopping and managing finances
  • Home helps: can provide assistance with general domestic tasks, including cleaning and cooking, and may be particularly important in maintaining hygiene in the home
  • Adaptations to the home: major adaptations (eg, installation of a stair lift or downstairs lavatory, or lowering work tops in the kitchen); minor adaptations (eg, handrails in the bathroom)
  • Meals: daily delivery of a meal, delivery of a weekly or monthly supply of frozen food or providing meals at a day centre or lunch club
  • Recreational, occupational, educational and cultural activities: day centres, lectures, games, outings and help with living skills and budgeting. This usually also involves providing transport to attend facilities

Home Modification

Home modifications ranging from simple home adaptions such as a grab rail, through to more complex modifications that involve structural changes, help participants be safer and more independent in their homes. Making these changes to homes can have a big impact on how people live their lives, allowing greater flexibility and maximising participant independence and safety.
Home modifications are changes to the structure, layout or fittings of a participant’s home so they safely access it and move around comfortably in areas they frequently use.

Home modifications may be included in your plan if:

  • Due to the impact of your disability, you or your carers cannot reasonably access and use frequently used rooms and spaces in your primary residence
  • Your primary residence, in its current condition, has significant and adverse impacts on the sustainability of current living and care arrangements
  • Suitably qualified Occupational Therapist has performed an assessment and recommended home modifications, considering all possible alternatives, including the use of equipment

Transportation

If you've lost mobility either on a temporary or long term basis, we can provide support including:

  • Door to door transport to and from your destination
  • Flexible pick-up times to suit your needs
  • Transport to your regular medical appointments, visiting loved ones and family celebrations

Social & Community

The NDIS gives people with disability choice and control over the supports they receive. It helps people with disability to live life their way, achieve their goals and participate in the community.

The Total Support Services team has vast experience in sourcing and coordinating fun, stimulating social and community activities. Certainly, sometimes there’s simply nothing better than a day spent away from home and exploring new places. In other words, you can do something very different from your every day or add a new activity to your routine. This service aims to support people with disability to achieve your goals in the community. See below the list of funded support by NDIS plan:
